Heterologous heart valve cusp transplantation


Abstract
A study on heterologous heart valve cusp transplantation was done in 28 dogs. Pig pulmonary artery valve cusp was transplanted into canine abdominal aorta as a heterologous patch graft.
Ten dogs expired before and immediately after operation due to cardiac arrest during anesthesia and bleeding after operation. Fifteen long-term survivors and three short-term survivors were obtained.
One, two, three, and four months post-operative long-term survivors were sacrificed after transfemoral retrograde abdominal aortography.
The following observations were made.
(A) Aortography findings:
In every aortogram patency was noted without obstruction or steno4.s. There were some cases which showed dilatation of the patched area.
(B) Macroscopic findings:
In one month specimens thin loosely attached membrane on the vascular luminar surface was noted. Most of the anastomosis sutures were still exposed. But on adventitial surface adhesion with surrounding host tissue was dense. Minor hematomas were noted around anastomosis sites.
In two months group most of the anastomosis sutures were covered and intimal lining was thicker with still rough surface.
In three months group luminar surface membrane was smooth and glistening, all suture materials were covered completely by newly formed tissue.
in four months group grafted patch could hardly be differentiated from surrounding host tissue.¢¥Intimal lining was complete..
(C) microscopic findings: =1 In one month group massive neutrophile leucocyte infiltration and some macrophages were noted in and around the graft.
In two months group small round cell and `macrophage infiltrations were pronounced with decreased leucocytes.
In three months group the aboye mentioned acute or chronic inflammatory finding¢¥s were very slight. Connective tissue proliferation along the luminar surface and around anastomosis sites were remarkable.
In four months group endothelialization was complete in the intimal site and graft was fully incorporated into the surrounding host aorta.
